The information is required to ensure that the County Administration System which All County F. A.’s are operating incorporates information which the F. A. requires to formulate a comprehensive Database relative to All football and in particular Grassroots Football.
No monies are to be submitted with this form at this stage as an Invoice will subsequently be issued to each applying club indicating the payment to be made to the D. C. F. A. Forms are to be returned by 1st June 2011 and the subsequent invoice will be payable 14 days from date of invoice. On receipt of these payments a Clubs Affiliation Number will be issued.

It should be noted however, that clubs affiliating a number of sections within the same club name will be allowed the same affiliation number for each section provided that the Secretary, Chairperson and Treasurer are the same for each section. This means that correspondence from the D. C. F. A. will be directed to one point of contact for all sections be they Saturday or Sunday or midweek.
Clubs when indicating other Leagues in which their teams play must also indicate which day the League plays. The League Division in which an Adult team will be participating must be stipulated.
Each Invoice submitted will also include £5.00 Beneficiaries Fund donation.
Further to this I have to advise that the Football Association has issued a Directive requiring that from the commencement of Season 2007-08 it is mandatory for ALL clubs to have a Public Liability Insurance Scheme in place through a club’s parent County F. A.
The fee for providing Civil Liability including Public Liability Insurance Scheme will be incorporated in each Clubs Affiliation Fees and the sum will be £30.00 per club (NOT per team). Irrespective of how many teams a club operates under the same affiliation number the fee will remain at £30.00 per club.
The provider will be Sportguard and the Indemnity Limit will be £5,000,000.
The scheme does NOT include Player to Player injury insurance, that aspect of insurance and the provision of such will remain with each individual club.
Once affiliated the Policy Schedule will be issued to the Secretary of the Club. Clubs will not be allowed to opt out of the scheme nor allowed to exempt themselves other than those clubs at the highest levels of the National Leagues System i.e. Step 4 and above.
Any form submitted that is incomplete will be returned and a financial penalty imposed.
